Our client is a doctor and the project was to provide a easier way for new patients to complete their paper work before coming in to the office.  We developed the required web pages for patients to fill up the form and convert the form as pdf and email it to the doctors back office.

Challenges: We had to use the existing template which is PDF and we had to map and write the fields to the existing pdf rather than creating new formats.
